Grade 3 Gynecomastia – 28-Year-Old Patient

Procedure: Bilateral gynecomastia surgery

Patient: 28 years old

Severity: Grade 3 gynecomastia

Technique: Liposuction and gland removal

Grade 3 gynecomastia often includes significant breast volume and loose skin. Skin excision may be recommended when the skin is unlikely to retract sufficiently, but this creates additional scars.

For this patient, skin elasticity and the trade-off between contour and scarring were discussed before surgery. Liposuction and gland removal were performed without immediate skin excision. Skin retraction can continue during recovery, especially in younger patients, but the degree of improvement cannot be predicted and additional skin treatment may still be considered if needed.

The surgical plan is individualized, and results vary by anatomy, skin quality, healing, and postoperative care.
