Why does Dr. Jeon prefer areolar incsion in gynecomastia surgery?

prefers the peri-areolar incision (side of nipple) rather than the incision in the armpit or lower chest.

Of course, underarm incisions are the least noticeable scars.

However, I do not prefer this incision method
except for special cases such as pseudogynecomastia.

Anyway, in gynecomastia surgery,
the key is to remove the mammary gland.

In true gynecomastia,
removal of the mammary gland is the most important point,
and this is the most technically difficult part.

This glandular tissue is a very complex organ.
Not only does it have a passageway connected to the nipple,
but it also attaches to the surrounding skin and chest muscle.

An underarm incision may be advantageous if liposuction alone is performed.
However, if mammary glands are removed,
there are too many disadvantages.

In particular, the area called the nipple-areolar complex requires detailed manipulation
because it is a complex of mammary ducts, blood vessels, and nerves.

Incomplete mammary gland removal may cause recurrence or decrease cosmetic satisfaction.

Most notably, for this reason,
Dr. Jeon prefers per-areolar incision.
