Nipple & Areolar Reduction Guide

Recovery after nipple reduction or areolar reduction surgery is usually straightforward, but careful protection of the surgical area is important during the first few weeks. Following these instructions can help support healing, minimize complications, and improve scar quality.

Most Important Notes

  • Absolutely no smoking for 3 weeks. Smoking reduces blood flow and may interfere with healing.
  • Protect the nipple and areola. Avoid unnecessary pressure, friction, or trauma to the surgical area.
  • Wear your compression garment as instructed. Consistent compression helps reduce swelling and supports healing.
  • Avoid strenuous activity. Increased heart rate and blood pressure may increase swelling and bleeding.

The First Night After Surgery

  • Rest quietly at home.
  • Limit activity to bathroom visits only if possible.
  • Drink plenty of fluids.
  • Avoid unnecessary movement.
  • Have assistance available if needed.

Medications

  • Take all prescribed medications exactly as instructed.
  • The first 3–5 days are usually the most uncomfortable period.
  • Swelling and discomfort may increase during the first few days before gradually improving.
  • This is a normal part of healing.

It is common to feel slightly worse on day 3 or 4 than on day 1 or 2. This does not necessarily indicate a problem.

Diet

  • Start with clear liquids or soft foods on the first evening.
  • Examples include soup, juice, crackers, or soft meals.
  • If tolerated well, gradually return to your normal diet.
  • Stay well hydrated during recovery.

Showering and Incision Care

  • Keep the surgical area clean and dry for the first 48 hours.
  • You may usually shower beginning on the third day after surgery.
  • Allow water to run gently over the area.
  • Do not scrub the incision.
  • If Steri-Strips are present, leave them in place.
  • After showering, allow the Steri-Strips to air dry naturally.
  • Do not soak the incisions in a bathtub, hot spring, swimming pool, or ocean until completely healed.

Sleeping Position

  • Sleep on your back during the first week.
  • Keep your upper body elevated approximately 30–45 degrees.
  • Placing a pillow under your knees may improve comfort.
  • Avoid sleeping on your stomach.
  • Avoid placing pressure on the nipples or areolas.

Activity Restrictions

Day After Surgery

Light Walking

Walk indoors every few hours while awake. Short walks may help reduce the risk of blood clots and support circulation.

First Week

Rest and Recovery

Focus on rest, hydration, nutrition, and gentle daily activity only.

Weeks 2–3

Gradual Return to Daily Activities

Light non-strenuous activities may gradually resume.

First 3 Weeks

No Strenuous Exercise

Avoid activities that significantly raise your heart rate or blood pressure.

First 6 Weeks

No Heavy Lifting

Do not lift, push, or pull objects heavier than approximately 5 kg (10 lbs).

Compression Garment

Compression garments help reduce swelling and support healing.

  • Wear the garment as close to 24 hours per day as possible.
  • Continue for approximately 3–6 weeks unless instructed otherwise.
  • You may usually remove it briefly for showering.
  • Replace it immediately afterward.
  • If the garment feels excessively tight, adjust it so circulation is not restricted.

What Is Normal During Healing?

  • Moderate swelling.
  • Bruising.
  • Tenderness.
  • Temporary changes in nipple sensation.
  • Temporary asymmetry caused by swelling.
  • Mild firmness around the surgical area.

These changes usually improve gradually over the following weeks and months.

Follow-Up Visits

  • Your first postoperative visit is usually scheduled approximately 1 week after surgery.
  • Sutures are commonly removed 10–14 days after surgery.
  • Additional visits may be scheduled according to your healing progress.

Scar Healing

Scar maturation is a slow process.

  • Scars usually appear pink or firm during early healing.
  • Scar appearance gradually improves over time.
  • Final scar maturation may take up to 2 years.
  • Consistent scar care may help improve scar quality.

  • Incisions that appear to be opening.
  • Increasing redness, warmth, or pus.

Nipple and areolar reduction surgery generally involves a relatively quick recovery, but protecting the surgical area during the first few weeks is essential.
